Student Video Scholarship Contest
Dale Frank, President of Dale A. Frank Insurance Agency, Inc announces a Student Video Scholarship Contest to introduce his new online 'Virtual Insurance Office' at www.DaleFrankInsurance.com. The new online website was produced to supplement the agency's live office staff in providing 24/7 information and services for current and prospective insurance and financial clients.
What is the scholarship contest about?
Because the internet has become an increasingly important source of information for anyone planning to purchase products as well as services, there is a need for accurate information. Our live staff provides the expertise you desire for choosing the best insurance coverages and prices, in addition to on-going services such as billing issues, claims and policy changes, all while working in conjunction with our new Virtual Insurance Office at www.DaleFrankInsurance.com. Students are asked to create a 30 - 60 second video showing why you would use our new online Virtual Insurance Office in conjunction with the live services of our agency located in Sunderland, MA. Using creativity and Originality, your video should effectively communicate, with aesthetic appeal, the services of Dale. OFFICIAL RULES:

Prizes: $1,000 cash prizes will be awarded by 11/29/2010. The Winners will be paid directly if age 18 or older and where the winner is under the age of 18, the parent or guardian, as custodian for the Winner. Any and all federal and MA income taxes are sole responsibility of the Winner and/or his or her parent or guardian. Sponsor will issue a tax form 1099 to the Winner. Payments of the award will be contingent upon verification that the applicant meets the necessary eligibility requirements.
CONTEST DATES:
The scholarship contest will start March 15, 2010 at 12:00pm EST and submissions will be due on 11/22/2010. Voting closes on 11/26/2010. The Winner will be announced on 11/29/2010.
Winner will be chosen at the offices of Dale A Frank Insurance Agency, Inc 2 Amherst Rd, Sunderland MA. Winner will be selected based on number of online votes received.
WHO IS ELIGIBLE:
The contest is open to high school students in grades 11 or 12 and anyone currently enrolled in College who are residents of MA, USA and age 13 or over at the time of entry. Person's under 18 years of age at the time of entry must receive the parents' or guardians' permission and provide parental/guardian contest in addition to a parents'/guardians' valid email address before entries will be considered. Employees, officers, directors, agents, representatives and their immediate families, advertising, design, web design, web hosting, printers, and anyone connected to the production and distribution of this scholarship contest are not eligible.
ENTRIES:
- Complete an application, available at the virtual insurance office, www.DaleFrankInsurance.com. Visit the contest page to complete an online entry form and agree to the terms and conditions of these rules. Students under the age of 18 years of age also must submit a parental consent form with a parents valid e-mail address. Anyone under the age of 13 is ineligible.
- Submit or post you're video on www.YouTube.com anytime between now and 12:00pm EST on 11/22/2010.
- Entry on YouTube to be made by the authorized account holder for the person assigned an email address by an internet online service provider or other organization responsible to assigning email addresses or domain associated with the email address. In the case of students less than 18 years of age, the parent or guardian will be the authorized email account holder. Only one entry per student and email address. Entries become the exclusive property of the sponsor and will not be acknowledged or returned.
Sponsor assumes no responsibility for lost, late, misdirected or unintelligible applications or videos.
JUDGING CRITERIA AND SELECTION OF WINNERS:
Submitted videos should be in good taste and will not be accepted or eligible if they include any content that contains or appears to contain:
Explicit profane or obscene materials, references or endorsements of illegal drug use, alcohol abuse or any other illegal activity, nudity, profanity or extreme violence, any express or commercial endorsements, any derogatory characterization of any person or group based on age, race, color, gender, sexual orientation, religious beliefs, marital status, political affiliation or other unethical or unlawful factors. Depictions of any conduct, language or other context deemed inappropriate by the sponsor or any judges.
Winner will be chosen at the offices of Dale A Drank Insurance Agency, Inc 2 Amherst Rd, Sunderland MA. A panel of objective judges selected by the sponsor will evaluate all eligible contest entries.

Publicity: USE OF SUBMITTED VIDEO, NAME and LIKENESS
Except where prohibited by law, entry in the Scholarship Contest constitutes each applicant's consent to the Sponsor's use of his or her name, city and state of residence, entry, likeness, photograph and image for advertising, publicity and promotional purposes in any media, online or offline without further permission, consent, payment or other consideration.
It is the responsibility of the Applicant to obtain release from any persons’ appearing in the video that are identifiable, whose name, voice or likeness is shown or otherwise used. Anyone under the age of 18 depicted in the video must have a release signed by their parent/guardian. Submitted videos may be disqualified at any time if the Sponsor finds that the Applicant has not obtained the proper releases.
GENERAL CONDITIONS:
The contest is governed by the laws of Massachusetts.
If any technical problems, or any other event beyond its reasonable control, compromise the integrity of the contest the Sponsor may cancel, modify or suspend contest without notice. In the event of cancellation winners will be selected from the applications received prior to the cancellation.
LIMITS OF LIABILITY:
Each applicant acknowledges and agrees that the Sponsor and related companies or affiliates and their related offices, director, agents and respective employees are not responsible directly or indirectly for any costs, personal injuries or other unnamed damages of any kind arising from participation in the Scholarship contest. Sponsor assumes no responsibility to an applicant for loss or damage to submitted material including hardware and software failures, malfunctions or error caused by the Sponsor or other unnamed organizations that provide electronic computer access to any and all website. Void where prohibited.
